Following the memorial service, local coroner William Bludworth (Tony Todd) mysteriously warns the survivors that they cheated Death. They ignore his warnings and move on with their lives.
Later, Candice goes to gymnastics practice with Peter, only to be killed when she snaps her spine after flipping off the highbars. The next day, Isaac is killed when his head is crushed by a falling Buddha statue during an acupuncture session at a Chinese spa. Bludworth, who has been present for both deaths so far, tells the remaining survivors that if they wish to cheat Death, they must each kill someone who was never meant to die on the bridge, and thereby claim their remaining lifespan. Sam and Molly suspected that Olivia is next and she's in danger, who has gone for laser eye surgery. They arrive at the clinic but are too late to see her fall to her death from a window onto a car's windshield. Later, after Sam and Molly studied Death's design, they notice that Nathan is next on Death's list.
Meanwhile, Nathan, who has returned to the plant, accidentally kills his antagonistic co-workerRoy (Brent Stait) during an argument between the two when he shoves him in the path of a hook, impaling his head. Nathan relays this information to the remaining survivors, who suggest that it means Nathan was successfully able to claim Roy's remaining lifespan and thus was skipped over. When Dennis arrives to question Nathan about the incident, he is suddenly killed when a stray wrench is launched by abelt sander and penetrated through his head.
That evening, Sam's mentor allows him to become an apprentice in Paris. He allows Sam to have the restaurant to himself for a date with Molly. Peter, who has now been driven paranoid and insane by Candice's death, interrupts the date in order to inform them that he nearly pushed a stranger in front of a truck after convincing himself that he would be able to kill someone else in order to take their lifespan; subsequently, he has decided to kill Molly and take her remaining lifespan for himself. After drawing a gun and firing shots, Sam and Molly both escape to the restaurant's kitchen. Nearby outside, Agent Block (Courtney B. Vance) overhears the shots and enters the restaurant. He arrives during the confrontation, but is shot and killed by Peter. Believing he is now safe from Death for taking Block's lifespan, he decides to kill both Molly and Sam to remove any witnesses. After some struggling, Sam kills Peter with a meat spit before he can harm Molly.
Two weeks later, Sam and Molly are boarding a plane to Paris. As they are taking their seats, a fight breaks out between two passengers, revealed to be Carter Horton and Alex Browning. As their argument irritates the other passengers being kicked out of the plane. The plane that Sam and Molly are boarding was revealed to be Flight 180 (revealing the film to be a prequel to Final Destination). The plane starts to break apart in mid-air, realizing that they are too late to save themselves, both are killed in the resulting order.
Meanwhile, at Roy's memorial, Nathan learns from a co-worker that Roy had a brain aneurysm that doctors said could have killed him at any moment. As Nathan realizes the repercussions of this information, the flaming landing gear from Flight 180 crashes through the roof of the building and crushes him.

BiographyEdit
Olivia was resided in New York. She is very promiscuous, provocative, insensitive and flirtatious. She works as an office assistant in the local sales division at Presage Paper. She is one of the employees qualified to go on a corporate retreat.
Final Destination 5Edit

After Candice's death, she is seen at work. She approach Sam and Molly about the grievance pay, and talks about Candice's death. She nearly felt happy when Candice is dead. Olivia then joins the others to have drinks before heading to the local clinic.
DeathEdit
When Olivia goes to have LASIK eye surgery, her head is tightly secured and her right eyelid pried open with a speculum. She squeezes a teddy bear out of nervousness, popping off its glass eye onto the floor. While Leonetti prepares her eye, he is missing file information and leaves her alone to consult his secretary.
A cup of water that Olivia left on top of a water cooler is knocked over when the cooler bubbles, and the cup spills onto the power unit of the laser machine plugged into the wall. The intensity of the ray overheats and the laser machine starts to shake. As Olivia reaches for the emergency stop button on the control, she knocks it to the floor, where the activation button is pressed, discharging laser in her eye.
As she shakes, the laser burns lines across her iris and pupil. She removes the speculum from her eye and holds her hand in the way, burning lines across her palm, although the laser still burns through her hand. Sam and Molly rush into the front office to save her from danger. As Leonetti assures she'll be fine, her screams are heard and they run to save her.
She manages to release her head from the clamp and get off the table before the laser fires again, her eye bloody and burnt. Sam, Molly, and Leonetti rush into the room, finding Olivia screaming. However, being in high heel boots, Olivia trips over the glass teddy bear's eye on the floor and stumbles out the window, falling about several stories onto a car parked out in the street below. Her dislodged eye rolls out onto the road only to be run over by a car.
Signs/CluesEdit
- Many signs of Olivia's death have to do with eyes, cars and falling:
- Before Olivia leaves, she knocks over her photo frame which make a crack pointing at her right eye. The picture shows that Olivia had been inMcKinley Park with her friend.
- The doctor asks her, "What made you decide to take the leap today?"
- The advertisement to the lasik center says "See the beauty of life"
- The necklace she wears is in an oval-shape with the center as an empty hole.
- The view of the laser device up close strangely resembles an eye.
- While Sam, Molly and Olivia were talking, there is a toy car in Sam's cubicle.
- When her eye came out, it was run over by another car.
- When she was talking about Candice Hooper when she died, she says "Guess we should have seenthat coming". The sentence is similar to how Tim Carpenter from Final Destination 2 was asked by his dentist "Your mom said you have got some pane (pain) lately. And they both died with falling (Tim got crushed by falling glass pane , and She fell down from the surgeon)
- During Olivia's death, A teddy bear replicates hers:
- When Olivia dropped down the teddy bear, it fell off the similar fashion as how she fell off the building.
- When she squeezed the teddy bear tight, it's right eye popped out in a similar fashion to hers.
- Olivia stepping on the removed eye of the teddy bear foreshadows the car running over her own removed eye.
- When she is getting burned by the laser the teddy bears are looking at her just like when Tod from Final Destination is getting strangled and the bath toys are looking at him.
- When she is starting to lay down for the procedure a dark shadow and a ominous wind blows over he

While practicing with her gymnastics team, Candice complains about the extreme heat of the gymnasium, urging her coach to turn on the air conditioning. When the air conditioning unit is finally activated, it shakes from over use, causing a screw to fall out and land sharp-side up on the balance beam. Although Candice has several close calls with the screw, she misses it and it goes unnoticed. As she practices on the high beams, another gymnast, Porter is practicing on the balance beam which steps on the screw and falls off the beam, knocking over a bowl of chalk in the process. The powder is blown into a fan and temporarily blinds Candice while she is flipping on the high beams. The chalk powder gets on her body and distracted and unable to see, she lets go of the bar and flips in mid-air, landing on her neck, while the powder preventing her from slipping had caused her spine to snap and various bones to protrude through her body, in front of Peter, the coach and the other gymnasts.
Signs/CluesEdit
- Candice feels a gust of wind blow past her.
- Her good-luck rubber band that she twirls on her wrist snaps prior to beginning practice, hinting at the nature of her death.
- During the conversation between Peter and herself, Peter repeatedly mentions that the practice is the "last one" for Candice before going to regionals.
- Candice says her coach would kill her if she missed the practice.
- During her practice, right when she moves to the bars, her coach tells her to "stay loose".
- Candice was almost electrocuted by stepping on the puddle that had dripped onto an exposed wire, but she was interrupted and went on to practice the high beams.
- In the premonition, her death is similar to Perry's from Final Destination 3.
- Both in the premonition and afterwards, Candice dies by support beams. In the premonition, she is impaled by the beam/pole if the boat that holds onto the sails. In her practice, she is moving on the unfixed, loose beam.
- Both in the premonition and afterwards, Candice falls to her death with her spine being damaged. In the premonition, she falls and is impaled through the spine. In her practice, she falls on the ground, breaking her spine.
- When Candice felt a gust of wind blowing by, a girl was using the balance beams in the background. If you look closely, it looks like that the girl is going to hit Candice's neck , like how Candice later broke her neck and spine.
- She is the only survivor to be mainly sports player (not a strong person, like Lewis Romero). Both of them died in, ironically, their gym.
- Candice's body was similar to Tim's, as both of them had snapped their back.

He then went to the bathroom at the back of the bus to have privacy. When Sam witnessed the premonition of the bridge falling apart, he freaked out and left the bus with Molly, then Isaac came out from the bathroom earlier, and he got off the bus too. DeathEdit
After the deaths of his fellow employees that did not escape the accident, Isaac raids their office cubicles. In Robert's drawer, he finds a coupon to a free spa therapy session. When he arrives at the Ming Yun Spa, he sexually harasses the young, attractive receptionist. Disgusted, she decides to take him to receive acupuncture from an old, unattractive woman in a room full of candles and incense. When he walks in, he rubs a Buddha statue sitting on a shelf on the wall for good luck. His cell phone, which he uses to call multiple women, is taken from him by the masseuse and placed on a shelf next to a lit candle. The masseuse performs rough, violent body massage techniques on Isaac, which causes the bolts on the leg of the table he is laying on to come undone. After the masseuse finishes the acupuncture on him, she leaves him to sleep for thirty minutes.

aflame. Isaac sees this, and in a panic stumbles off the table, causing the legs to give way, sending him to the floor face down. A large bottle of alcohol is knocked over (used to sterilize the needles) and covers him and the floor. Isaac gets up painfully, with the needles stuck into his body and face, and tries to pull one out. During this distraction, Isaac's cell phone begins to vibrate, slowly moving the candle next to it off the shelf and into the alcohol. The alcohol is set ablaze and Isaac stumbles into the far wall of the room, untouched. As he whimpers in relief, the shelf above him holding the heavy Buddha statues gives way and the statue completely crushes his head.Signs/CluesEdit
- Dennis announces at the funeral that Isaac was one of the casualties on the bridge, even though he was alive and well.
- When Isaac is looking through the deceased employee's drawers, he finds one full of thumb tacks which could allude to the acupuncture needles.
- He pricks his finger like Sam did.
- A gust of wind makes all the candles flicker.
- When going into the room, he jokingly insults the Buddha statue that later kills him. Specifically, he comments on Buddha being fat, and it is the statue's weight that kills him. Because of that, his death is a form of divine punishment.
- While walking towards the room, he knocks on the walls, making sure that they are sound proof.
- He rubs Buddha's belly, which is something to do to get good luck.
- The spa that Isaac went called Ming Yun Spa. In Chinese, Ming Yun (命運) means fate or destiny.

Dennis is first seen needing his employees ready to go on the corporate retreat. When the bus the coworkers are on is on the North Bay Bridge, Sam, one of the employees, has a premonition of the bridge falling apart and manages to get some of his coworkers as well as Dennis off the bridge. Dennis later made a speech at the memorial of those who did not survive the incident. On the other hand, Lapman, who doesn't seem to care much about his employees, kept in contact with Agent Jim Block to monitor violent or suspicious behavior from his employees after the bridge collapse. As Peter told Dennis about the deaths of Isaac and Olivia, Dennis couldn't believe it as he thought Isaac was already dead and showed no concern or sorrow for Olivia's demise. At one point though, Dennis shows a bit of a struggle in losing some of his employees. When he is on the phone with an executive he replies that one person would send the executive the papers. But, the executive reminds Dennis of that person's death. The executive asks Dennis if he wants someone to fly in to help out around the office, Dennis declines. After the call Dennis lays his head down on his desk in frustration.

After Roy's accidental death, Dennis came into the factory to investigate and encounters Nathan, Sam, Peter, and Molly. As Sam tries to figure out who is next to die, the vibrations from the machine eventually knocked the wrench into the rapidly spinning belt, projecting it at Dennis's face in a flash. The wrench was shot horizontally, penetrating his head in front of the survivors.Signs/CluesEdit
- Dennis says that it's usually the factory workers who get laid off. He died in the factory.
- Also, he tries to solve/fix several issues by contacting Agent Block. He dies by the wrench that is meant to fix things.
- Peter says that Dennis could be the next person to die, this was after Nathan was skipped so it was Dennis's turn.
- The wrench flew very close to Nathan's head; any closer and it would have been Nathan killed by it. The fact that it killed Dennis instead lends credence to the theory pertaining to the nature of being skipped from Death's list.
